Washington, DC, United States (AHN) – First time claims for jobless benefits for the week ending July 23 crept down slightly to 398,000, which was just below the critical 400,000 mark for the first time since April.

That number was down by 24,000 from the prior week’s figure of 422,000.

The less volatile 4-week moving average was 413,750, down by 8,500 from the previous week’s revised average of 422,250, according to the U.S. Department of Labor.

In terms of the total number of people claiming benefits in all programs, that number went up slightly for the week ending July 9, which is the latest week such data is available. Some 7,645,601 nationwide were collecting jobless benefits then, which was up by 320,152 from the previous week.

Washington, DC, United States (KaiserHealth) – When hospitals discharge patients, they typically see their job as done. But soon, they could be on the hook for what happens after Medicare patients leave the premises, and particularly if they are readmitted within a month.

In an effort to save money and improve care, Medicare, the federal program for the elderly and disabled, is about to release a final rule aimed at getting hospitals to pay more attention to patients after discharge.

A key component of the new approach is to cut back payments to hospitals where high numbers of patients are readmitted, prodding hospitals to make sure patients see their doctors and fill their prescriptions.

Medicare also wants to pay less to hospitals with higher-than-average costs for patient care. It has proposed calculating the costs by combining a patient’s hospital expenses with fees incurred up to 90 days after discharge.

The efforts, called for in last year’s health care law, are part of a push to make hospitals the hub for coordinating care. Hospital care is the largest chunk of Medicare spending; Medicare says readmissions alone cost $26 billion in a decade. Plus, many experts argue hospitals are the most organized actors in a splintered and often dysfunctional health system, and thus best able to take the lead in overseeing patient care.

But hospital groups complain that Medicare’s plans could punish them for things they can’t control, such as unavoidable readmissions and patients who can’t afford the costs of prescriptions.

“A lot of this is very unfair,” says Blair Childs, a vice president at Premier, an alliance of more than 200 hospitals. He says hospitals that don’t have a lot of money to invest in improving their oversight of former patients could end up losing more money under Medicare’s proposals, putting them in an even bigger financial hole. In particular, he says, the changes may hurt inner city hospitals.

“These are often very stressed hospitals, and they’re the ones that are going to be penalized the most,” Childs says.

Some academics who have studied hospitals also think Medicare may be being too harsh. “The truth is the 30-day readmission is a relatively lousy quality measure for a hospital, because a lot is happening outside a hospital’s control,” says Dr. Ashish Jha, a professor at Harvard’s School of Public Health.

Medicare’s penalties could be significant-and widespread. Almost 7 percent of acute-care hospitals — 307 out of 4,498 — had higher-than-expected readmissions rates for heart failure, heart attack or pneumonia, according to Medicare data. Under Medicare’s draft proposal, which it put out in May, penalties would start in October 2012; hospitals with the worst readmissions rates eventually could lose up to 3 percent of their regular Medicare payments.

Hospitals with patients who cost Medicare lots of money during and after their hospital stays also could be hurt. Beginning in October 2013, these spending levels would count for a fifth of Medicare’s “value-based purchasing program,” which alters hospital payments based on long list of quality measures.

“The incentives we’re putting into place have created a whole new way to think about hospital care,” says Jonathan Blum, deputy administrator of the federal Centers for Medicare & Medicaid Services, or CMS.

These initiatives come on top of other Medicare experiments that will make not just hospitals but also surgeons responsible for costs run up from complications that occur beyond the operating room. One approach is “bundled payments,” where Medicare pays a set fee for the entire cost of a patient’s treatment, including expenses after discharge. And Medicare’s high-profile venture to create “accountable care organizations,” where teams of doctors and hospitals share the financial risks and rewards for caring for patients, would also hold hospitals partially to account for the costs of treatments that patients get elsewhere.

CMS has limited leeway to tinker with the readmissions rule, because much of it was spelled out in the health care law. CMS has more freedom to change its plan to measure per-patient spending, as the law didn’t detail how it should work.

Regardless of what CMS ultimately decides, many hospitals are already scrambling to change how they supervise former patients, says Chas Roades, chief research officer at The Advisory Board Company, a health care consultancy. “One of the big themes I’m hearing now across the hospital industry is, ‘We can no longer think of ourselves as just hospital companies, we have to be full-service health care managers,’” he says.

Consider Trinity Health, which owns 50 hospitals around the country, including Holy Cross in Silver Spring, Md. Before patients leave the hospital, Trinity’s nurses now set up appointments for them with their regular doctors. They also make sure patients can get to the appointment, either by helping them figure out whether Medicare or Medicaid pays for transportation, or by paying for the trips directly.

“We’re trying to do a better job of sending them home better-prepared, rather than just saying good luck,” says Dr. Terry O’Rourke, Trinity’s chief clinical officer. But he says there are limits to what they can do: “The majority of physicians are not employed by the hospital, and we don’t have control over their practices.”

Dr. Kavita Patel, a Brookings Institution fellow and former Obama administration official, says changes occurring in both the private sector and Medicare will speed up the trend of hospitals’ overseeing the care of former patients.

For example, she says, many hospitals are buying the practices of primary care doctors, making it easier for them to arrange and oversee the care of patients after discharge. “The more hospitals realize they’re going to be held accountable, that’s where they are going to get creative,” Patel says.

London, England, United Kingdom (AHN) – Reports that came out on Thursday said that Britain would lay off 7,000 civilian workers from its Ministry of Defense. The axing of jobs is part of the national austerity measures to reduce Britain’s record-high budget deficit.

It specifically is in the Strategic Defense and Security Review made in October, in which thousands of employees would lose their jobs, the Harrier fleet and Nimrod spy aircraft would be scrapped and the HMS Ark Royal aircraft carrier retired.

The 7,000 to become jobless is just about a fourth of the 25,000 civilians scheduled to be axed from the service by 2015.

The downsizing would result in civilian post in the ministry whittled to 60,000 workers.

Reports said that the soon-to-be removed staff would receive a letter signed by the ministry’s permanent secretary, Ursula Brennan.

Aside from reducing its civilian employees, the coalition government also announced last week that its regular armed forces would be cut to its smallest size in a century. However, the government would increase its reserves in the next decade.

British Defense Secretary Liam Fox said last week that with a target total force of 120,000, 70 percent would be full time regular soldiers and 30 percent part time members of the Territorial Army.

Reports of the axing of 7,000 civilian ministry employees angered union leaders and defense officials, who said they were not consulted about the move. Those who would lose their jobs are mostly engaged in going over contracts to ensure that the agency does not exceed its budget.

Annandale, VA, United States (AHN) – There was tense drama at the Annandale campus of the University of Northern Virginia (UNVA) on Thursday as law enforcement agencies raided the premises.

Mirroring the Tri Valley University (TVU) scam, UNVA’s permit to admit foreign students was put on immediate hold, creating a dilemma for the school’s international students, including a majority from India.

Officials of the US Immigration and Customs Enforcement (ICE) and Federal Bureau of Investigation (FBI) entered the UNVA campus and seized all official documentation, computers, hard drives and even waste paper baskets.

“I was just teaching in my class. The police opened the door. Three different police officers’ group, ICE, FBI, with all their paraphernalia. They told us to turn off my phones and stay in our rooms while they secured the building,” said Diana LaVora, a teacher at the school.

Jonathan Cooper, another instructor, said, “I came to work tonight to teach evening ESL class and found that the class is not in session and that all the documentation from the University of Northern Virginia — the administrative portion has been removed — international student documentation pertaining to their visas — their admission status by the ICE and the FBI.”

“At this point it seems the most important thing is to helping the students understand what the next steps are and that appears to be calling the Resource Center from the Student Exchange Program and that will determine what their next steps are,” he added.

During the day, while the raid was going on, ICE spokeswoman Cori W. Bassett said in a statement, “Today, officials from ICE’s Student and Exchange Visitor Program (SEVP) served UNVA with a Notice of Intent to Withdraw (NOIW) UNVA’s authorization to admit foreign students. UNVA students should call the SEVP Response Center at 703-603-3400 for guidance.”

UNVA is an unaccredited private university located in Northern Virginia. Founded in 1998, the university claims accreditation by the American University Accreditation Council (AUAC), which is not a recognized accreditation agency in the United States.

Fort Washington, PA, United States (AHN) – The company that makes the pain reliever Extra Strength Tylenol is lowering its recommended daily dose of the drug.

McNeil Consumer Healthcare Division announced Thursday that the dosing instructions on the package will change from eight pills (4,000 mg) to six pills per day (3,000 mg). The move is in response to the medication’s active ingredient acetaminophen, which has been linked to liver damage.

Dr. Edwin Kuffner, vice president of OTC Medical Affairs and Clinical Research at McNeil, said acetaminophen is safe when used as directed, but liver damage can occur if a person takes too much.

“McNeil is revising its labels for products containing acetaminophen in an attempt to decrease the likelihood of accidental overdosing in those instances,” Kuffner said in a statement.

The new dosing instructions will appear on Extra Strength Tylenol packages beginning in the fall.

London, England, United Kingdom (AHN) – The hard times had changed the attitude of workers, particularly when it comes to switching jobs. According to a survey of 2,000 employees by the Chartered Institute of Personnel and Development, more than 50 percent of the workers said the reason they would look for new employment is because of higher pay.

In 2010, the majority of the employees identified job satisfaction as the main reason why they look for new work.

The study explained the shift in reason to pay increases that are less than inflation rate and salary freezes. The regular yearly pay increase, excluding bonuses, is at 2.1 percent, which is half of the current inflation rate in Britain.

Because of these developments, 36 percent of the survey respondents said their standard of living had worsened for the past half year. Only 7 percent said their situation has improved.

Besides seeking higher-paying jobs, the same employees on the lookout for better wages are also likely to report that they are stressed at work or not satisfied with their current employment.

The report also disclosed that job satisfaction has decreased in the public sector, where civil servants said they had high levels of job insecurity.

Aside from suffering redundancies, public sector workers got a lower rate of basic increase at an average of only 0.9 percent compared to the 2 percent given to private employees, according to an earlier report by the institute.
